Improve model card: add paper link, GitHub link, and summary

This PR improves the model card by:
- Linking the model to the associated research paper: Genetically Aligned Patient Representations Improve Hematological Diagnosis.
- Providing a link to the official GitHub repository.
- Adding a description of the two training stages (vision-only pretraining vs. genetic alignment).
- Including a code snippet for downloading the checkpoints as found in the GitHub documentation.
